Mometasone Furoate 0.1% w/w:Mometasone furoate (C27H30Cl2O6, molecular weight 521.4) is a synthetic diester corticosteroid classified as a medium-to-high potency topical glucocorticoid, characterised by a favourable pharmacological profile that delivers potent local anti-inflammatory action with comparatively lower systemic absorption than older-generation topical steroids. Following application to affected skin, mometasone furoate diffuses through the stratum corneum and binds with high affinity to intracellular glucocorticoid receptors within dermal and epidermal cells. The activated receptor-ligand complex translocates to the cell nucleus, where it modulates gene transcription upregulating anti-inflammatory proteins such as lipocortin-1 (annexin A1) and downregulating the expression of pro-inflammatory mediators including prostaglandins, leukotrienes, histamine, and cytokines. This transcriptional modulation blocks the arachidonic acid cascade at its upstream phospholipase A2 step, effectively curtailing the entire inflammatory signalling network that causes the redness, oedema, pruritus, and epidermal thickening characteristic of dermatitis and eczema flare-ups. Fusidic Acid (C31H48O6) typically 2% w/w:Fusidic acid is a steroidal antibiotic derived from the fermentation product of the fungus Fusidium coccineum, belonging to the fusidane class of antibiotics. It targets Gram-positive bacteria with particular potency against Staphylococcus aureus including methicillin-sensitive strains which is the primary bacterial pathogen in most companion animal secondary skin infections. At the molecular level, fusidic acid exerts its bactericidal effect by binding to and stabilising the complex formed between elongation factor G (EF-G), the ribosomal GTPase involved in bacterial translocation, and GDP on the bacterial ribosome. By locking this complex in a non-productive conformation, fusidic acid prevents the ribosomal translocation step essential for protein chain elongation during bacterial translation. The result is complete inhibition of bacterial protein synthesis, starving the organism of the structural proteins, enzymes, and regulatory molecules essential for cellular function, growth, and reproduction leading to bacteriostasis and, at higher concentrations, bactericidal cell death. Fusidic acid achieves excellent penetration into infected skin tissue and retains activity in the presence of wound exudate and purulent material, making it particularly effective in superficial and dermis-level skin infections.
